- Geographic location of the tree (This could be a post code, map reference, GPS location etc.)
- Where was the tree found?
- Was there any debris under the tree from last years dead leaves?

I undertook the survey in my neighbour’s garden. Post code:- LD2 3UL.

OS No:- O 0/14; 078474

There were no leaf mines on any of the ten leaflets examined.

There was no debris from last years dead leaves.

There were no other Horse-chestnut trees with 100 paces of that tree.

I tried to complete the on-line survey, but it kept telling me to use whole numbers where I entered ‘0’.
